Precision Medicine ～ Delivering the right treatments, at the right time, every time to the right person ～

先月、アメリカでPrecision Medicineについて話を聴いてきた。恥ずかしながら、そのときまで、あまりPrecision Medicineについて理解していなかった。あまりの無知ゆえに、分からないことが出てくる度に、メモ代わりにtwitterでつぶやいていた。

色々と内容がつながってきたタイミングで、下のオバマ大統領の演説を見て、アメリカが国を挙げて医療を変えようとしていることがひしひしと伝わってきた。この動画は、一度見ておいても損はないと思う。

細かな内容は下記にまとまっている。

主な投資内容・投資額を述べた箇所を引用する。

Key Investments to Launch the Precision Medicine Initiative:Complementing robust investments to broadly support research, development, and innovation, the President’s 2016 Budget will provide a $215 million investment for the National Institutes of Health (NIH), together with the Food and Drug Administration (FDA), and the Office of the National Coordinator for Health Information Technology (ONC) to support this effort, including:

• $130 million to NIH for development of a voluntary national research cohort of a million or more volunteers to propel our understanding of health and disease and set the foundation for a new way of doing research through engaged participants and open, responsible data sharing.

• $70 million to the National Cancer Institute (NCI), part of NIH, to scale up efforts to identify genomic drivers in cancer and apply that knowledge in the development of more effective approaches to cancer treatment.

• $10 million to FDA to acquire additional expertise and advance the development of high quality, curated databases to support the regulatory structure needed to advance innovation in precision medicine and protect public health.

• $5 million to ONC to support the development of interoperability standards and requirements that address privacy and enable secure exchange of data across systems.

以下、上記内容の意訳。

2016年の大統領予算で、2億1,500万ドル（約260億円）をNIH、FDA、ONCに投資する

• 1億1,300万ドル（約140億円）はNIHにおける100万人以上のボランティアによる遺伝子コホート研究のために

• 7,000万ドル（約80億円）はNCIにおけるがんの遺伝子特定、がん治療の研究のために

• 1,000万ドル（約12億円）はFDAにおけるデータベース整備等に

• 500万ドル（約６億円）はONCにおけるセキュアなデータ交換などの規約整備のために

100万人以上の遺伝子情報を解析するという話はアメリカの講演でも聞いていたが、背景には、このオバマ大統領の話があったのか・・・と、今更ながらに理解した。

なお、客観的な見方としては、下記の動画も参考になる。

Precision Medicine。高精度医療とでも呼べばいいのだろうか。医師の勘に頼った医療から、エビデンスに基づく医療・EBM（Evidence Based Medicine ※医師の経験や患者本人の意志を尊重することは大前提）へ進化し、その延長線上にPrecision Medicineがあると考えるべきだろう。これは一部の研究者だけが知っていれば良いことではない。アメリカの状況を鑑みると、国民の理解と協力が不可欠だからだ。日本で良い医療を受けたければ（さらに大きな枠組で考えれば、日本の競争力を向上・維持するためには）、協力しなければならないだろう。

余談だが、遺伝的な繋がりを考えると、アジア圏での協力も重要なことは言うまでもないだろう。
